К вопросу о значении специфических антигензависимых и неспецифических факторов в патогенезе хронической трансплантационной нефропатии

Аннотация: Патогенез хронической трансплантационной нефропатии (ХТН), которая является основной причиной отдаленных потерь трансплантатов, во многом не ясен. Полагают, что в патогенезе имеют значение как антигензависимые, так и антигеннезависимые факторы. В настоящем исследовании изучены некоторые механизмы возникновения и прогрессирования ХТН. Среди последних специальное внимание уделено роли клинических синдромов - артериальной гипертонии (АГ) и протеинурии, а также характерных морфологических признаков ХТН - тубулоинтерстициального склероза (ТИС), гломерулосклероза и хронической трансплантационной гломерулопатии (ХТГ). Результаты исследований показали, что в механизме возникновения ХОТ играют роль как антигензависимые, так и антигеннезависимые неспецифические факторы. Среди антигензависимых факторов патогенетическое значение имели неадекватность иммуносупрессии и кризы отторжения, но лишь те из них, которые отличались рефрактерностью к массивной иммуносупрессии и купировались только частично. Из неспецифических патогенетических факторов наиболее тесную связь с последующим возникновением ХТН имели: дисфункция трансплантата через 6 мес. после АТП, АГ и протеинурия. Прогрессирование ХТН определялось, главным образом, уровнем креатинина плазмы в момент биопсии, выраженностью АГ и распространенностью ТИС. При этом действие каждого из этих факторов было независимым. Значение гломерулярной патологии до прогрессирования ХОТ оказалось не столь очевидным. Если гломерулосклероз оказывал значимое влияние на темпы прогрессирования ХТН, то наличие ХТГ ухудшало прогноз лишь при сочетании с выраженным ТИС. Полученные данные позволяют считать, что в отличие от пусковых механизмов ХОТ важнейшими факторами его прогрессирования являются антигеннезависимые, опосредованные прогрессирующим нефросклерозом.
