Интегрированный подход к заместительной почечной терапии

Аннотация: Проанализирован опыт лечения 298 пациентов, находившихся на перитонеальном диализе (ПД) в Московском городском нефрологическом центре (ГКБ № 52) в 1995-2002 гг., и литературные данные. ПД является важной составляющей заместительной почечной терапии (ЗПТ). В течение первых 5 лет лечения его эффективность не уступает эффективности гемодиализа. Современная ЗПТ предполагает интеграцию разных методов лечения. При этом ПД следует рассматривать как оптимальный метод для начала ЗПТ, после которого больным может быть выполнена трансплантация почки или лечение гемодиализом. С другой стороны, пациенты гемодиализа при отсутствии возможности поддержания адекватного сосудистого доступа или при серьезной, гемодинамически значимой сердечной патологии могут быть переведены на перитонеальный диализ. Такой комбинированный подход позволяет значительно продлить жизнь пациента с терминальной стадией хронической почечной недостаточности.
